The Ottawa Township High School, in conjunction with OSF Healthcare, will hold a sports physical and baseline concussion testing clinic Tuesday, Aug. 2.
The testing will run from 1-7 p.m. and take place at the school, 211 E. Main St., Ottawa. The cost is $25, and both cash and check will be accepted. If you need financial assistance, contact the Ottawa High School Athletic Department at 815-433-1323.
Physicians, athletic trainers and nurses will be on site. Students should bring a completed consent form and a completed Illinois High School Association form, both of which are available at the school's website at www.ottawahigh.com.
You can schedule your appointment by calling 815-431-5233, but walk-ins will also be accepted.
Proceeds from this event will be donated to the Ottawa High School Athletic Program.
This sports physical does not meet the requirements for the incoming freshmen's school physical.
