Illinois Valley Community Hospital issued the following announcement on Oct. 18.
October 31 is the last day to complete Illinois Valley Community Hospital’s online health needs survey, a key part of the community health needs assessment IVCH conducts every three years.
The link to the survey can be found at the top of the home page of the hospital’s web site, www.ivch.org. The link will remain live until the end of October.
Paper copies of the survey and postage-paid envelopes in which to return them are available at IVCH and at IVCH Medical Group offices in LaSalle, Peru, Oglesby, Utica, Princeton and Streator.
The triennial survey helps IVCH determine where to best allocate its resources to help Illinois Valley residents maintain access to healthcare services
