BUREAU VALLEY COMMUNITY UNIFIED SCHOOL DISTRICT 340: Sports Physical Clinic May 21st!

Bureau Valley Community Unified School District 340 recently issued the following announcement.

A sports physical clinic will be held at Bureau Valley High School on Monday, May 21, 2018 beginning at 5:15 p.m. for all Bureau Valley incoming seventh, eighth, sophomores, juniors and seniors. 

Incoming fifth grade boys participating in JFL may also attend this clinic. 

Area doctors are providing the community service for any Bureau Valley student who needs the required sports physical. 

This clinic is NOT for incoming sixth grade students or incoming freshman students as they need a complete school physical.

Students need to sign up and pick up a sports physical packet from their school office. 

This packet consists of the sports physical form, concussion protocol form, insurance form, IHSA sports medicine form and random drug testing form. 

Students MUST bring the completed packet, including a parent’s signature on all forms, and $10.00 the night of the clinic, or bring a parent with them. 

Sports physical packet forms can also be found on the school website. Note: IHSA and IESA rules do not recognize or accept physicals from chiropractors.
